Building Use Requests - First Baptist Church sponsored events requiring use of the building must be approved through the office to help avoid scheduling conflicts and schedule janitorial staff.  Turn in the Calendar Request form to the church office for approval.      

Scholarships  - Church members attending a Southern Baptist College or University are eligible to apply for a church scholarship or the Amelia White Memorial Scholarship.  Forms and requirements are available for print here: Scholarship Application

Youth Medical Release -  Youth church activities involving travel require a medical release to be updated annually.  Note:  Falls Creek requires a separate medical release.  The current year medical release can be printed here and turned into the church office to be kept on file for youth events:  Youth Medical Release
